Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your primary insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.

Cancellation Policy

Because we have allotted time and space for you on our schedule for a given day, we ask that if you confirm your appointment with the office at least TWO business days in advance. 

If your appointment is not confirmed within 24 hrs of your scheduled time, your surgery may be canceled to accommodate other patients.

If you need to cancel or reschedule your surgical appointment, we require 48 business hours in advance of your appointment time, to avoid a $275 cancellation fee.
